Introducción: Streptococcus pneumoniae causa enfermedades graves en la población susceptible. La vacuna neumocócica conjugada (PCV) 13-valente (PCV13) se incluyó en el calendario infantil en 2011. Este estudio analiza la evolución de los serotipos de neumococo y de sus resistencias tras la PCV13. Métodos: Se incluyeron los neumococos serotipados en Galicia en 2011-2021. Se estudió la sensibilidad antibiótica siguiendo criterios EUCAST. Se analizaron los datos en 3 subperíodos: inicial (2011-2013), medio (2014-2017) y final (2018-2021). Se calcularon las prevalencias de los serotipos y el porcentaje de resistencia a los antibióticos más representativos. Resultados: Se incluyeron 2.869 aislados. Inicialmente el 42,7% presentaba tipos capsulares incluidos en la PCV13, frente al 15,4% al final. Los incluidos en la PCV20 y no en la PCV13 y PCV15 fueron el 12,5% inicialmente y el 41,3% al final. El 26,4% de los serotipos a lo largo del estudio no estaban incluidos en ninguna vacuna. La prevalencia del serotipo 8 se multiplicó casi por 8 y la del 12F se triplicó. El serotipo 19A fue el más resistente inicialmente. La resistencia de los serotipos 11A y 15A aumentó a lo largo del estudio. Conclusiones: La introducción de la PCV13 en la población infantil determinó un cambio en los serotipos de neumococo hacia los incluidos en la PCV20 y los no incluidos en ninguna vacuna. El serotipo 19A inicialmente fue el más resistente, y el 15A, no incluido en ninguna vacuna, merece un especial seguimiento. El serotipo 8, que fue el que más se incrementó, no mostró resistencia destacable.(AU)

The objective of the study was to analyze the evolution of pneumococcal serotypes and their resistance after PCV13. Methods: This study included the pneumococci serotyped in Galicia in 2011-2021. Antibiotic susceptibility was analyzed following EUCAST criteria. The data was analyzed in 3 sub-periods: initial (2011-2013), middle (2014-2017) and final (2018-2021). The prevalence of serotypes and their percentage of resistance to the most representative antibiotics were calculated. Results: A total of 2.869 isolates were included. Initially, 42.7% isolates presented capsular types included in PCV13, compared to 15.4% at the end. Those included in PCV20 and not in PCV13 and PCV15 were 12.5% at baseline and 41.3% at the end; 26.4% of the isolates throughout the study had serotypes not included in any vaccine. The prevalence of serotype 8 multiplied almost by 8 and that of 12F tripled. The 19A serotype was initially the most resistant, while the resistance of serotypes 11A and 15A increased throughout the study. Conclusions: The introduction of PCV13 in the pediatric population determined a change in pneumococcal serotypes towards those included in PCV20 and those not included in any vaccine. Serotype 19A was initially the most resistant and the 15A, not included in any vaccine, deserves special follow-up. Serotype 8, which increased the most, did not show remarkable resistance.(AU)

(1) Background: Insufficient physical activity in adolescents remains an important issue for health promotion. Given the current relevance of understanding the adoption and maintenance of moderate and vigorous physical activity (MVPA), the aim of this study was to analyze, in a sample of adolescents, the role of grit personality as an antecedent of healthy eating and healthy weight (HEW) self-efficacy and its implications for the practice of MVPA. (2) Methods: Participants were 987 adolescents (597 girls, 390 boys) aged between 15 and 19 years from Mexico and Spain. The Spanish versions of the grit personality scale, the healthy eating and weight self-efficacy scale and the global physical activity questionnaire were used to measure the variables of interest. (3) Results: Mediated regression analysis showed that grit personality was not directly related to MVPA practice. However, the results indicate the significant relationship between grit personality and HEW self-efficacy, as well as the positive and significant relationship of this self-efficacy on MVPA practice. HEW self-efficacy totally mediated the relationship between grit personality and MVPA in both boys and girls. (4) Conclusions: These results suggest that having a grit personality (i.e., having interest and perseverance) is not enough for adolescents to be physically active, but that perceiving oneself as effective in having a healthy diet and healthy weight may be the key for adolescents to move more. At the intervention level, we suggest targeting an enhancement of young people's competence to eat healthily and regulate their weight as a strategy to enhance the performance of more MVPA, with a possible transfer between healthy behaviors (spill over).

BACKGROUND: Research on procrastination, regarding time perspective factors and basic psychological need satisfaction (BPNS) has placed this problem at the meeting point of individual and contextual variables. The present study focused both on the individual, given that time perspectives can be defined as a person's attitude to an object (time) at three moments (present, past, future); and on contextual aspects, because the satisfaction of basic psychological needs (competence, autonomy, relatedness) is facilitated or made difficult by social contexts. Based on this, the aim of this study was to analyse the relationships between time perspectives and inter-subject procrastination variations, testing the moderating role of BPNS in this relationship. METHOD: A total of 1,188 undergraduate students, aged 17-50 years (M = 20.02, SD = 2.63), completed three questionnaires containing the variables of interest. RESULTS: Regression analyses showed significant negative (thus, potentially protective) association of future time perspective with all three procrastination dimensions (decisional procrastination, implemental delay, and lateness). Conversely, past-negative time perspective showed a positive (thus, potentially adverse) association with procrastination. Satisfaction of the need for competence also showed a negative (thus, potentially protective) association with all procrastination dimensions. On eight occasions, the relationships between time perspectives and procrastination dimensions were moderated by psychological need satisfaction. CONCLUSIONS: These findings show that BPNS may play relevant roles in the negative (favourable) relationships between procrastination dimensions and positive time perspectives, as well as in the positive (adverse) associations between negative time perspectives and procrastination dimensions. Contextual interventions fostering enhanced levels of perceived autonomy, competence, and relatedness, as well as future time perspective, are thus strong candidates to consider for use and evaluation by policy makers, pedagogues, teachers, coaches and other professionals interested in counteracting procrastination tendencies.

Introduction: This study aims to understand how emotions and motivation influence the academic achievement of physical education (PE) students and their future intention to practice physical activity (PA). Despite the influence on student's behaviors and the reciprocal associations between motivation and emotion, the number of studies addressing both constructs at the same level is very limited. Methods: A structural equation model was used with 799 students aged 11-17 years (M = 13.16; SD = 1.17). Results and discussion: The results showed that the teacher support of the basic psychological needs (BPN) predicted students' BPN satisfaction, which in turn predicted their autonomous motivation and positive emotions, and negatively predicted their negative emotions. Finally, autonomous motivation predicted students' intention to be physically active, whereas academic achievement was predicted by both autonomous motivation and emotions. We conclude that to better understand the consequences of PE classes, it is necessary to consider both constructs.

Background: There is a need to understand better factors influencing participation in physical education (PE) and the mechanisms involved. The adolescent years are characterised by increasing levels of body-related concerns. In PE, the body is judged for its physical abilities and subject to social comparisons and body judgements. Grounded in the Self-Determination Theory, this study aimed to explore whether body-related factors were associated with adolescents' involvement in PE and whether types of motivation mediated this relationship. Methods: The study involved 2,140 (54.5% girls) secondary students (15-16-year-olds) from Norway participating in the nationally representative "Health Behaviour in School-aged Children (HBSC) study: a WHO collaborative cross-national study." Body-related factors included Body Mass Index (BMI), health complaints, body perception and dietary behaviours. Gender, age, and socioeconomic status (family affluence) were control variables. Motivation for PE was assessed with the Perceived Locus of Causality (PLOCQ) scale measuring three distinct factors: autonomous motivation, controlled motivation and amotivation. PE involvement was self-reported as weekly participation in PE classes and time spent in moderate-to-vigorous physical activity (MVPA) during PE. Results: Gender (girl), family affluence, health complaints, not being on a diet but wanting to lose weight, and body perception (too fat) were negatively associated with weekly PE participation when adjusting for other variables. This association was largely explained by students' autonomous motivation in the case of health complaints and partly in the case of dietary behaviour and body perception. Similar results were observed for MVPA during PE lessons. Additionally, gender was associated with MVPA through amotivation. Conclusion: The study adds new knowledge to the understanding of the relationship between body-related factors and PE, supporting that autonomous motivation is a central mechanism and an avenue for further research. The results should be considered in planning high-quality PE classes and suggest that an autonomous supportive learning climate sensitive to body-related concerns should be a priority to increase adolescent involvement in PE.

The aim of this study was twofold: first, to examine the stability of the hypothesized conceptual model of motor development (without and with including various types of motivation) when children are followed up one-year later, and second to examine longitudinally whether changes in one model variable predict changes in other variables, according to the hypothesized pathways in the model. A sample of 361 Spanish students (50.7% girls, 8-11 years old) voluntarily participated in this study. In relation to the first aim, structural equation modeling revealed the expected positive relationship between the model variables in both measurement times. That is: actual motor competence (MC) predicted physical activity (PA) (p < .001), perceived MC mediated the relationship between actual MC and PA (p < .001), and autonomous motivation mediated the relationship between perceived MC and PA (p < .05). Moreover, the comparison of the invariance analysis showed non-practical differences between the unconstrained model and the constrained model, supporting the stability of the model over time. In relation to the second aim, the hypothesized model in Time 2 controlling for Time 1 values showed that changes in children's actual MC positively predicted changes in their perceived MC (p < .001), which in turn, predicted changes in their autonomous motivation (p < .001), and PA (p < .001) at Time 2. Based on these findings Physical Education teachers are recommended to foster children's actual and perceived MC as well as their autonomous motivation over time in order to promote PA strategies for lifelong health.

Physical literacy is crucial for children's appropriate development and physical, social and mental health. In a school setting, class-based physical activity (PA) interventions are considered to be appropriate programs to foster PA participation and cognitive development. The purpose of this project, named the Active Learning in PHYsical Literacy (ALPHYL) study, was to describe a multicomponent classroom-based physically-active learning randomised control trial (RCT) in primary school children. The main purpose was to promote children's physical literacy, academic achievement and cognitive function. The ALPHYL study is mainly based on physical literacy, active school models and the Supportive, Active, Autonomous, Fair and Enjoyable principles. The ALPHYL is an 8-10-week RCT to be conducted in six primary schools (12 classes) in Valencia (Spain) and its metropolitan area. Schools will be randomly assigned to the intervention or waiting-list control group. After a 30 h in-person training course for teachers and weekly meetings in the three months of resource preparation, the ALPHYL intervention will be conducted in physical education (PE) and non-PE lessons by teachers. The intervention consists of at least three daily sessions of physically active learning in addition to model-based PE teaching. Its feasibility will be evaluated weekly according to the Reach, Effectiveness, Adoption, Implementation and Maintenance framework. To assess its effectiveness, a pre-test, post-test and retention (8-10 weeks post-intervention) with primary outcomes (i.e., PA level, motor competence, perceived motor competence and PL, motivation, perceived social support, academic achievement and cognitive function), secondary outcomes and covariates will be collected.

Rock-based materials exposed to outdoor environments are naturally colonised by an array of microorganisms, which can cause dissolution and fracturing of the natural stone. Biocolonisation of monuments and architectures of important cultural heritage therefore represents an expensive and recurring problem for local authorities and private owners alike. In this area, preventive strategies to mitigate biocolonisation are generally preferred to curative approaches, such as mechanical cleaning by brush or high-pressure cleaning, to remove pre-existing patina. The aim of this work was to study the interaction between biocidal polyoxometalate-ionic liquid (POM-IL) coatings and calcareous stones and evaluate the capacity of these coatings to prevent biocolonisation through a series of accelerated ageing studies in climate chambers, carried out in parallel with a two-year period of outdoor exposure in north-eastern France. Our experiments show that POM-IL coatings did not affect water vapour transfer nor significantly alter the total porosity of the calcareous stones. Simulated weathering studies replicating harsh (hot and wet) climatic weather conditions demonstrated that the colour variation of POM-IL-coated stones did not vary significantly with respect to the natural uncoated stones. Accelerated biocolonisation studies performed on the weathered POM-IL-coated stones proved that the coatings were still capable of preventing colonisation by an algal biofilm. However, a combination of colour measurements, chlorophyll fluorescence data, and scanning electron microscopy imaging of stones aged outdoors in northern France for two years showed that coated and uncoated stone samples showed signs of colonisation by fungal mycelium and phototrophs. Altogether, our results demonstrate that POM-ILs are suitable as preventative biocidal coatings for calcareous stones, but the correct concentrations must be chosen to achieve a balance between porosity of the stone, the resulting colour variation and the desired duration of the biocidal effect over longer periods of time, particularly in outdoor environments.

El objetivo del estudio fue explorar la asociación entre el estilo de liderazgo transformacional de entrenadores/as y la identidad social de los/as deportistas, explorando el papel mediador de la satisfacción/frustración de las necesidades psicológicas básicas de los/as deportistas. Participaron 524 jugadores de baloncesto (325 chicos, 180 chicas) con edades comprendidas entre los 11 y los 17 años (M = 13.85; DT = 1.56) pertenecientes a diferentes clubes de la provincia de Valencia (España). Los resultados mostraron asociaciones significativas entre todas las variables del estudio. Asimismo, se confirmó el papel mediador de la satisfacción de las necesidades psicológicas básicas entre el liderazgo transformacional del entrenador/a y la identidad social de los/as deportistas. La mediación de la frustración de dichas necesidades no resultó significativa. Los resultados resaltan la importancia de los entrenadores/as en el desarrollo psicosocial del equipo, y apoyan la necesidad de formarles en el uso de estilos de liderazgo transformacional, que incidan sobre la satisfacción de las necesidades psicológicas básicas para promover la identidad social de los y las deportistas. The aim of this study was to explore the association between coaches’ transformationalleadership style and athletes’ social identity, exploring the mediating role of satisfaction/frustration of athletes’ basic psychological needs. Participants were 524 basketball players (325 boys, 180 girls) aged between 11 and 17 years (Mage= 13.85; SD= 1.56) belonging to different clubs in the province of Valencia (Spain). The results showed significant associations between all the study variables. Furthermore, results confirmed the mediating role of the satisfaction of basic psychological needs between the transformational leadership of the coach and the social identity of the athletes, whereas the mediating role of the frustration of the basic psychological needs was not significant. The results highlight the importance of coaches in the psychosocial development of the team, and support the need to prepare them in the use of transformational leadership styles, which affect satisfaction of basic psychological needs to promote the social identity of the athletes. O objectivo do estudo era explorar a associação entre o estilo de liderança transformacional dos treinadores e a identidade social dos atletas, explorando o papel mediador da satisfação/frustraçãodas necessidades psicológicas básicas dos atletas. Um total de 524 jogadores de basquetebol (325 rapazes, 180 raparigas) com idades compreendidas entre os 11 e os 17 anos (Midade= 13.85; TD= 1.56) de diferentes clubes da província de Valência (Espanha) participou. Os resultados mostraram associações significativas entre todas as variáveis do estudo. Do mesmo modo, foi confirmado o papel mediador da satisfação das necessidades psicológicas básicas entre a liderança transformacional do treinador e a identidade social dos atletas. A mediação da frustração destas necessidades não foi significativa. Os resultados salientam a importância dos treinadores no desenvolvimento psicossocial da equipa, e apoiam a necessidade de os treinar na utilização de estilos de liderança transformacional, que influenciam a satisfação das necessidades psicológicas básicas para promover a identidade social dos atletas. INTRODUCTION: Immunotherapy has become a standard treatment for lung cancer; the objective of this study was to evaluate the effectiveness, safety of pembrolizumab monotherapy in patients with advanced or metastatic non-small-cell lung cancer used in real-world clinical practice. MATERIAL AND METHODS: Retrospective observational study of every patient treated with pembrolizumab in our centre from January 2017 to June 2019. Outcomes collected: sex, age, Eastern Cooperative Oncology Group, programmed death receptor 1 level, previous metastatic line therapies, adverse events and smoking status. RESULTS: A total of 62 patients were reviewed. The median age was 62.34 ± 10.62 years, 48 (77.41%) were men and 91.93% of patients had Eastern Cooperative Oncology Group 0. The median dose administered was 170.5 mg (108 - 240 mg) and the median follow-up was 3 months (range: 1 - 38). A median of four cycles of pembrolizumab (range: 1 - 56) were administered as monotherapy. The reason for treatment discontinuation was mainly due to disease progression in 38.70% of patients or death in 30.64%. As first-line pembrolizumab monotherapy, median progression-free survival was 7.7 months (95% CI: 3.66 - 11.73) (N = 33). With respect to patients who were treated in second-third-line treatment, median progression-free survival was 3.5 months (95% CI: 2.40 - 4.59) (N=29). As to overall survival, pembrolizumab-treated patients as first-line treatment reached 19 months median OG (95% CI: 13.36 - 24.63) (N = 33) and those treated in second-third-line treatment got 11 months (95% CI: 3.4 - 18.5). A total of 64.51% of patients presented some adverse events to pembrolizumab however, only, 9.38% of them were grade 3. CONCLUSION: Pembrolizumab represents an effective and feasible alternative in terms of progression-free survival. It is a well-tolerated treatment option.

RESUMO

Bezold's abscess (BA) is a rare complication of otitis media that presents as a lateral neck abscess below the mastoid tip. BA incidence has recently decreased due to early diagnosis and prompt antibiotic intervention. We present a 42-year-old male with a complicated otitis media developing a 10 cm BA. Treatment of the lesion included surgical drainage and mastoidectomy, accompanied by intravenous (IV) broad-spectrum antibiotic administration. The patient experienced no adverse events during or after surgery and was placed on postoperative observation. However, on postoperative day (POD) 2, the patient left the hospital against medical advice and did not undergo further follow-up.

RESUMEN Introducción: José Martí, héroe nacional de Cuba, ha legado con el paso del tiempo, diversas ópticas progresistas, inclusive en el deporte, una faceta menos conocida en su fecunda labor periodística y literaria. Se conoce que practicó una sola especialidad: el ajedrez; pero en sus crónicas demostró conocimientos sobre carreras de fondo, fútbol americano, billar, boxeo, equitación, esgrima, caza, corridas de toros y patinaje, entre otros. Objetivo: Profundizar en las concepciones morales del Héroe Nacional de Cuba acerca del deporte y la Educación Física. Materiales y métodos: Para llevar a cabo este estudio se desarrolló una revisión bibliográfica de las obras completas de José Martí, referidas a las escenas norteamericanas donde se recoge en mayor medida las crónicas martianas acerca del deporte y la Educación Física. Resultados: El tema que se aborda por su interés trasciende las fronteras de su época y se inserta en el proyecto de las ideas que sustentan a la revolución cubana, de que el deporte es un derecho del pueblo; a ello va dedicado el presente artículo, a resaltar las opiniones que vertió José Martí sobre el deporte, la Educación Física y la Recreación, siendo un crítico de estos desde el punto de vista moral. Conclusiones: José Martí elaboró un arsenal de ideas que constituyen un alto aporte al desarrollo del deporte y la Educación Física. En el periodismo el maestro tuvo una de las visiones más certeras sobre la práctica del deporte.


SÍNTESE Introdução: José Martí, o herói nacional de Cuba, legou com o passar do tempo, várias visões progressistas, inclusive no esporte, uma faceta menos conhecida de sua prolífica obra jornalística e literária. Sabe-se que ele praticou apenas uma especialidade: xadrez; mas em suas crônicas ele demonstrou seus conhecimentos de corrida de longa distância, futebol americano, bilhar, boxe, equitação, esgrima, caça, toureio e patinação, entre outros. Objetivo: Estudar em profundidade as concepções morais do Herói Nacional de Cuba sobre esporte e Educação Física. Método: a fim de realizar este estudo, foi realizada uma revisão bibliográfica dos trabalhos completos de José Martí, referindo-se às cenas norte-americanas onde as crônicas de Marti sobre esporte e Educação Física são coletadas em maior escala. Resultados: O assunto tratado por seu interesse transcende as fronteiras de seu tempo e está inserido no projeto das idéias que sustentam a revolução cubana, que o esporte é um direito do povo; é a isso que este artigo é dedicado, para destacar as opiniões que José Martí expressou sobre esporte, Educação Física e Recreação, sendo um crítico destes do ponto de vista moral. Conclusões: José Martí elaborou um arsenal de idéias que constituem uma grande contribuição para o desenvolvimento do esporte e da Educação Física. No jornalismo, o professor tinha uma das visões mais precisas sobre a prática do esporte.


ABSTRACT Introduction: José Martí, Cuba's national hero, has bequeathed various progressive perspectives over time, including in sports, a lesser-known facet of his prolific journalistic and literary work. It is known that he practiced a single specialty: chess; but in his chronicles he demonstrated knowledge of long-distance running, American football, billiards, boxing, horse riding, fencing, hunting, bullfighting and skating, among others. Objective: To deepen the moral conceptions of the National Hero of Cuba about sports and Physical Education. Materials and methods: to carry out this study, a bibliographic review of the complete works of José Martí was developed, referring to the North American scenes where Martí's chronicles about sports and Physical Education are collected to a greater extent. Results: the topic that is addressed by his interest transcends the borders of his time and is inserted in the project of the ideas that sustain the Cuban revolution, that sport is a right of the people; This article is dedicated to this, to highlight the opinions expressed by José Martí on sports, Physical Education and Recreation, being a critic of these from the moral point of view. Conclusions: José Martí developed an arsenal of ideas that constitute a high contribution to the development of sport and Physical Education. In journalism, the teacher had one of the most accurate views on the practice of sport.

Ante el aumento del porcentaje de sobrepeso y obesidad a nivel mundial, el control de peso es una conducta primordial de promoción de salud. La presente investigación integra el modelo de personalidad Grit, la teoría de la autodeterminación y el modelo transteórico del cambio para tratar de comprender la conducta del control de peso: El objetivo del trabajo consistió en evaluar la personalidad Grit y la motivación como antecedentes psicológicos de las etapas de cambio para el control de peso. Participaron 1351 adultos de México entre 18 y 65 años, quienes cumplimentaron un paquete de cuestionarios con las variables de interés. Los resultados mostraron que la personalidad Grit se relacionó positiva y significativamente con las etapas de mantenimiento, acción, preparación y contemplación y de manera negativa y significativa con la etapa de precontemplación, a través de los tipos de motivación (autónoma, controlada y no motivación), sugiriendo que la personalidad Grit y la motivación son atributos importantes a tener en cuenta en el cambio hacia el control de peso. Este estudio provee de herramientas para la comprensión de la adherencia del control de peso a los profesionales implicados en la consulta nutricional y así promover un mejor abordaje interdisciplinar del sobrepeso y la obesidad.(AU)


Given the increasing percentage of overweight and obesity worldwide, weight control is a primary health-promoting behaviour. Inte-grating Grit personality, self-determination theory, and the transtheoretical model of change, this study evaluates Grit personality and motivation as psychological antecedents of the stages of change towards weight control. A total of 1351 Mexican adults between 18 and 65 years of age completed a packet of questionnaires on the variables of interest. The results showed that Grit personality was positively and significantly related to the mainte-nance, action, preparation, and contemplation stages, and negatively and significantly related to the precontemplation stage, through the types of motivation (autonomous, controlled, and amotivation),suggesting that Grit personality and motivation are important attributes to take into ac-count in the change towards weight control. This study provides tools to help professionals involved in nutritional consultation to understand weight control adherence, thus promoting a better interdisciplinary ap-proach to overweight and obesity.(AU)

OBJECTIVES: To know the impact of COVID-19 in incidence and lethality in nursing homes in Galicia. METHODS: This is a descriptive study of nursing homes residents and workers with confirmed COVID-19. The analysis spanned from March 1, 2020 to March 27, 2022, stratified into 6 periods (one per wave). The impact on incidence (attack rate, number of outbreaks, reinfections, sex, age, and diagnostic technique) and lethality (by sex, age, place of death, and number of centers with deaths) was analyzed. RESULTS: There were 15,819 people affected, 51.9% of the jobs and 47.0% of the workers. The attack rate in residents was: 5.8% in the first wave, 10.4% in the second, 6.3% in the third, 0.1% in the fourth, 2.1% in the fifth and 27.3% in the sixth. In the sixth wave, there were 11.3% reinfections and the number of outbreaks in was 3 times higher than in the second. The case fatality in residents was higher during the first wave (21.8%) and lower during the sixth (2.4%). He only had one worker in relation to COVID-19. CONCLUSIONS: Surveillance of COVID-19 in nursing homes was essential to understand the dynamics of the disease. The sixth wave was the one with the highest incidence and the lowest lethality. Lethality was higher in the first wave. The fourth and fifth waves had a lower incidence due to the effects of vaccination.

Objetivos: Conocer el impacto de la COVID-19 en incidencia y letalidad en los centros residenciales de mayores (CRM) de Galicia. Métodos: Se trata de un estudio descriptivo en residentes y trabajadores de los CRM con COVID-19 confirmada. El análisis abarcó del 1 de marzo de 2020 al 27 de marzo de 2022 y se estratificó en 6 períodos (uno por ola). Se analizó el impacto en incidencia (tasa de ataque, número de brotes, reinfecciones, sexo, edad y técnica diagnóstica) y letalidad (por sexo, edad, lugar de fallecimiento y número de centros con fallecidos). Resultados: Hubo 15.819 personas afectadas, el 51,9% de las plazas y el 47% de los trabajadores. La tasa de ataque en residentes fue: 5,8% en la primera ola, 10,4% en la segunda, 6,3% en la tercera, 0,1% en la cuarta, 2,1% en la quinta y 27,3% en la sexta ola. En la sexta ola hubo un 11,3% de reinfecciones y el número de brotes fue 3 veces mayor que en la segunda. La letalidad en residentes fue mayor durante la primera ola (21,8%) y menor durante la sexta (2,4%). Solo falleció un trabajador en relación con la COVID-19. Conclusiones: La vigilancia de la COVID-19 en CRM fue fundamental para conocer la dinámica de la enfermedad. La sexta ola fue la de mayor incidencia y la de menor letalidad. La letalidad fue superior en la primera ola. La cuarta y la quinta ola tuvieron menor incidencia debido a los efectos de la vacunación. (AU)

Protocadherin 19 (PCDH19) syndrome is inherited as an X-linked pattern and affects mainly females. This syndrome is caused by a mutation in the PCDH19 gene encoding for the protocadherin protein. It is characterized by refractory seizures during febrile episodes with neuropsychiatric manifestations. There is no consensus on the treatment of PCDH19. We conducted a literature review to investigate the main drugs used for this syndrome, and to evaluate the best possible course of adjuvant treatment for these patients. We used an advanced PubMed search strategy with the following inclusion criteria: a) full-text papers, b) English Language, and c) studies conducted in humans. Exclusion criteria: a) literature reviews, b) systematic reviews, and c) metanalysis. We gathered 26 observational papers to conduct this literature review on clobazam and bromide which have been shown to reduce seizures by 50%. Corticosteroids improved neurological symptoms during the episodes in a few patients. Nevertheless, they recurred after a few months. Preliminary results of ganaxolone, which is still under study, demonstrated a reduction of 60% in seizure episodes. A ketogenic diet has been studied to treat several refractory epilepsies, including PCDH19; it has promising results as effective adjuvant therapy in the resolution of status epilepticus, suggesting it could be used as part of the treatment in early childhood. Stiripentol was given as adjuvant therapy in a patient with PCDH19 epilepsy resulting in the most extended period of seizure-free episodes, but more studies must be performed to assess its efficacy.

BACKGROUND: With the emergence of SARS-CoV-2, influenza surveillance systems in Spain were transformed into a new syndromic sentinel surveillance system. The Acute Respiratory Infection Surveillance System (SiVIRA in Spanish) is based on a sentinel network for acute respiratory infection (ARI) surveillance in primary care and a network of sentinel hospitals for severe ARI (SARI) surveillance in hospitals. METHODS: Using a test-negative design and data from SARI admissions notified to SiVIRA between January 1 and October 3, 2021, we estimated COVID-19 vaccine effectiveness (VE) against hospitalization, by age group, vaccine type, time since vaccination, and SARS-CoV-2 variant. RESULTS: VE was 89% (95% CI: 83-93) against COVID-19 hospitalization overall in persons aged 20 years and older. VE was higher for mRNA vaccines, and lower for those aged 80 years and older, with a decrease in protection beyond 3 months of completing vaccination, and a further decrease after 5 months. We found no differences between periods with circulation of Alpha or Delta SARS-CoV-2 variants, although variant-specific VE was slightly higher against Alpha. CONCLUSIONS: The SiVIRA sentinel hospital surveillance network in Spain was able to describe clinical and epidemiological characteristics of SARI hospitalizations and provide estimates of COVID-19 VE in the population under surveillance. Our estimates add to evidence of high effectiveness of mRNA vaccines against severe COVID-19 and waning of protection with time since vaccination in those aged 80 or older. No substantial differences were observed between SARS-CoV-2 variants (Alpha vs. Delta).

University students represent a population that faces high risks regarding physical inactivity. Research suggests that a regular engagement in physical activity (PA) may be more likely established when it leads to the experience of subjective vitality. Subjective vitality, in turn, is more likely achieved through physical activities that individuals feel competent in, and that take place in natural outdoor environments. An activity that may fulfill these conditions is active commuting to and from university (ACU). To examine whether and in which form ACU can combine this promising pattern of aspects, a person-oriented analysis was conducted. The sample contained 484 university students (59.3% females). Leisure-time PA, ACU by walking, ACU by cycling, subjective vitality, PA-related competence and body mass index were included as input variables in a self-organizing maps analysis. For both female and male university students, the identified clusters indicated that students who intensively engaged in ACU did not exhibit subjective vitality levels above average. Consistently, they did not show elevated levels of PA-related competence, which suggests that ACU does not support the perception of their physical abilities. Considerations regarding urban university environments lacking sufficient natural elements finally add to the conclusion that engaging in ACU does not suffice to establish a vitality-supportive and thus sustainable PA behavior. Additionally, the identified clusters illustrate a large heterogeneity regarding the interaction between leisure-time PA, body mass index and subjective vitality.

The increasing resistance of pathogenic microorganisms against common treatments requires innovative concepts to prevent infection and avoid long-term microbe viability on commonly used surfaces. Here, we report the preparation of a hybrid antimicrobial material based on the combination of microbiocidal polyoxometalate-ionic liquids (POM-ILs) and a biocompatible polymeric support, which enables the development of surface coatings that prevent microbial adhesion. The composite material is based on an antibacterial and antifungal room-temperature POM-IL composed of guanidinium cations (N,N,N',N'-tetramethyl-N″, N″-dioctylguanidinum) combined with lacunary Keggin-type polyoxotungstate anions, [α-SiW11O39]8-. Integration of the antimicrobial POM-IL into the biocompatible, flexible, and stable polymer poly(methyl methacrylate) (PMMA) results in processable films, which are suitable as surface coatings or packaging materials to limit the proliferation and spread of pathogenic microorganisms (e.g., on public transport and hospital surfaces, or in ready-to-eat-food packaging).
